What is Divine Hospice Care?

Hospice care focuses on providing comfort and pain management, rather than providing a curative treatment. Hospice not only supports the patient but also their family and is offered in whatever setting is most comfortable – whether your home is a house, an apartment or an assisted living facility. Hospice is a family focused, team approach and Divine Hospice has a team of professionals here to care for the patient’s needs, as well as their loved ones.


What Makes Hospice Special?


Your care is provided by a team of professionals that work with you and your family to develop a personalized plan for your care. This team includes your doctor, hospice nurse, hospice social worker, a hospice aide, the hospice medical director, a volunteer and the hospice chaplain.

Each of these team members plays a vital role in making sure the end of your life is lived fully with dignity and comfort. At the center of the team is not only you, the patient, but also your family!

Whether your family includes relatives, friends, church members or neighbors, they are expected to actively participate in your care with the hospice team. Hospice is designed to assist your family members in providing comfort – physically, emotionally and spiritually.


Who is Eligible for Hospice Care?

Hospice can be provided to anyone, regardless of age, who has been diagnosed with a terminal illness and has been given 6 months or less to live (if the illness runs its normal course). We understand this is a very difficult and overwhelming time. Know that hospice will be there to support you and your family. Once you have made the decision to live the end of your life as fully and comfortably as possible, talk to your doctor about how hospice can help.
