Social Services

Social Work services are provided for any diagnosis or psychosocial crisis resulting from the stresses of disease on the individual patient and their family. Social workers serve our patients by making the transition from home to hospital smoother by utilizing their assessment skills to identify problem areas. Social Work Services may also be ordered by a physician for a homebound patient. A personalized treatment plan is then created to facilitate healing and provide a safer care environment with support for the patient.
