Your nurse will perform a comprehensive evaluation and will work closely with you and your doctor to develop an individualized plan of care that is in your very best interest.

Home Health Aide

Under the supervision of your RN, Home Health Aides assist with personal cares to help make daily living a little bit easier. They use techniques for a safe transfer to chairs, beds and couches. They also help with bathing, dressing and assist with bathroom needs. These are just a few of the services your Home Health Aides perform to help you live safely at home.

Physical Therapy

The goal of Physical Therapy is to help you improve your strength, flexibility, balance and coordination, which in turn will help you to:

  • Walk on level ground and navigate stairs and curbs

  • Conserve your energy and limit strain on joints and muscles

Occupational Therapy

The goal of Occupational Therapy is to help you regain the ability to perform everyday activities independently, such as eating, dressing, bathing, preparing a meal or enjoying your favorite hobby. Your Occupational Therapist will help you to:

  • Adapt your day-to-day routine to meet your abilities
  • Improve fine motor skills so that buttoning a shirt, putting on shoes or cutting food becomes easier
  • Live safely and comfortably in your own home

Speech Therapy

The goal of Speech Therapy is to regain and strengthen your speech and/or language skills and in some cases, improve your ability to eat and swallow. Strokes, certain developmental delays, neuromuscular diseases and neurological disorders can all affect your communication, language and your ability to eat safely. Your Speech Therapist will develop a program with strategies designed specifically for you.

IV Therapy

Divine Home Care offers IV therapy to assist you with prescribed medical treatments in the comfort of your own home. Your nurse works closely with your doctor to determine exactly what type of therapy is required and the length of the treatment in order to administer your medication at home.

Interpreting Services

It is your right to be involved in your plan of care by communicating any needs or concerns you may have, as well as what is going well for you. Divine Home Care will provide you with interpretive services to eliminate those language barriers if necessary.

Social Services

Social Work Services are provided for people during a transition period or when there is a need for additional help and resources.

Personal Care Assistance (PCA)

If you need assistance with daily activites in order to remain living at home, PCA services may be an option for you. PCA services are provided to you through the county you live in and you must qualify for these services by completing an assessment that determines your needs.

Home Care Nursing (HCN)

Home Care Nursing services provide assistance with the day-to-day tasks of life – 24 hours per day, 7 days per week. Care is provided for those who need hospital level nursingto maintain independence at home. These services are available for people of all ages, from your new born baby to a spouse or grandparent.
